Run an async callback across items with a fixed worker limit.

  Concurrency and cancellation options.
- fn: (item: T, index: number) => Promise<void>
  Async callback invoked once per item until completion or abort.
Returns Promise<BoundedConcurrencyResult>
Worker count and whether cancellation was observed.
